The Michigan Primary Care Association is the health center association in Michigan and a longtime partner of CareQuest Institute. MPCA is one of the partners originally involved in the Safety Net Solutions initiative that led state-based and national work to advance the integration of oral health into primary care. In Michigan, the PCA has supported a ton of progress over the past few years in moving health centers to value-based payment models on the medical side and are well-positioned and wanting to do something relatively similar on the dental side. MPCA is well-positioned as a leader in interoperability and has a robust measurement set to leverage and roll out to begin capturing how value can exist in both care and payment.

This proposed work builds on several months of collaborative work between the MPCA and the CareQuest Institute VBC Team. This collaborative proposal is considered a part of the next iteration of COrHT for CareQuest Institute.

The two high level phases of work that the MPCA seeks to implement through this process include:

Phase 1: Engaging and partnering with health centers to implement the CareQuest Institute VBC Readiness Assessment that the VBC team has successfully integrated into Qualtrics.

Phase 2: Assess the individual and aggregate data from the VBC Readiness Assessment and partner with the CareQuest Institute to design and implement a 6-month long COrHT learning collaborative.

The Learning Collaborative will strengthen the capacity of participating health centers to practice in a value-based environment, as well as operate with a strengthened data and interoperability capacity. This work will be built off the robust measurement framework that MPCA has designed, which creates an environment with greater capacity for interoperability and population health management, all focused on advancing new, value-based payment models.

The points of value for this proposal include that it maximizes a window of opportunity to advance oral health value-based care and payment. The results of this work will be foundational for MPCA to design a future pilot that will lead into broader adoption. As well, this work aligns with the trajectory of COrHT as one of our new cross-enterprise initiatives and will be the first instance of implementation through a cross-team (grants and VBC) collaboration.

The budget for this proposal is $200,000 which includes time and materials associated with implementing the deliverables. The budget includes $32,000 for a consultant who will support conversations between MPCA, the health centers, Michigan’s Medicaid program, and the health plans. The budget also includes a $35,000 contract partnership with the Michigan Oral Health Coalition to support the engagement of community members as well as advocacy efforts and stipends to the participating health centers.

This budget amount represents less than 1% of the total MPCA budget.
