Since at least 2015, through CareQuest Institute funding, the State University of Iowa (UI) has been leading efforts to build consensus around a design for the Patient Centered-Dental Home (PCDH), which centers on operationalizing oral health value-based metrics. The goal is to develop a framework for quality measurement and care integration within dentistry and between dentistry and the broader health home environment. This work has been conducted over several phases to build out the conceptual framework with a broad range of stakeholders involved.

The work to identify measures and standards that can be used to operationalize the framework is currently underway. The purpose of this proposed project is to complete the development phase of the PCDH framework and begin the implementation stage. To do this, the State University of Iowa will:

Finalize the development of PCDH measure sets that achieve standardization and alignment. A central focus of this work is designing measure sets that allow for flexibility and adaptability to different use cases and care settings.

Build the pilot phase framework, finalize pilot evaluation tools, design an implementation guide, and recruit pilot partners.

UI have been following a consistent framework as to what was used to build the PCMH model.

The value proposition for CareQuest Institute is supporting the finalization of the PCDH framework and the staging for the piloting and implementation phase of the work. The PCDH framework has a great deal of alignment with the project implementation work associated with both the MORE Care and COrHT initiatives and is focused on being equitable, evidence-based, and across the lifespan. The plans for piloting and broader implementation could create some distinct synergy within our future cross-enterprise initiatives as well as align with the 3 domains framework. Additionally, this framework could play a significant role in advancing and standardizing how oral health will be integrated into overall health care.

The budget for this proposal is $185,000, which primarily supports staff time associated with the deliverables and the design of the pilot framework and tools. This budget represents less than 1% of the total University budget.
