State University of Iowa is seeking funding to continue moving forward its Patient Centered Dental Home project. The goal of this project is to develop a patient-centered dental home model of care, similar to the patient-centered medical home, that acts as a framework for quality measurement and care integration within dentistry and between dentistry and the broader health home environment. The PCDH model aligns with and supports value-based care initiatives: it is prevention-focused, patient centered, and emphasizes care coordination with the broader healthcare system. The PCDH model is designed to: 1) promote improved oral health outcomes and patient care experiences across the lifespan; 2) be applicable in a range of settings; 3) advance health equity; and 4) integrate with other practice transformation initiatives in the overall healthcare system.

As of August 2023, the Patient-Centered Dental Home (PCDH) Model of Care has been developed, with over 500 clinical quality measures having been assessed, selected for the model and published. State University of Iowa's 2023 proposal moves from the conceptualization phase and into implementation.

To do this, within the proposal, State University of Iowa outlines the following activities:

Recruit three pilot Community Health Center sites for implementation of the Patient Centered Dental Home claims/Electronic Health Record measure sets. CareQuest Institute will have the opportunity to review pilot sites.

Create a PDCH learning collaborative comprised of pilot site partners, steering committee members, Medicaid agency staff, FQHCs and payers. This will serve as a space for ongoing learning and TA throughout the pilots.

Implement the claims/Electronic Health Record measure sets in the pilot sites by providing training, site visits, a Pilot Implementation Guide and ongoing support.

Evaluate the pilots and refine implementation resources to further assist pilot sites and future adopters.

The project will continue to be overseen by a National Advisory Committee and a steering committee.

The request was originally submitted at $350,000, almost double the granted amount in 2022. This was largely due to the addition of pilot site support to the budget. Philanthropy staff worked with University staff to reduce the budget to the recommended $275,000. Within this overall budget, $150,000 is allocated for staff time and evaluation. The remainder of the budget will be disseminated to pilot site partners and to the University for indirect expenses. The request represents less than 1% of the organization's overall budget.
