Our Grant Application Process

Through our grantmaking, we work with partners to catalyze oral health transformation. Together, we are guided by one vision: an oral health system that is just, equitable, and accessible to all.

Step 1: Review Funding Eligibility and Proposal Alignment

Organization:

All grantees must:

    • Have demonstrated experience in, or willingness to begin engaging in, policy or systems-change work at the local, state, or national level
    • Have a proven track record of applying a health equity or racial equity lens in their work
    • Be committed to deepening their expertise in the social determinants of oral health
    • Be a 501c3 tax exempt organization in good standing with the IRS, a government agency or an educational/research institution that is tax exempt.
    • Be based in the 50 US states or Washington, DC. All funded activities must occur within the 50 US states or Washington, DC.

    Approach:

    Proposals should be led by, or strongly informed through trusted relationships with, communities that have historically been affected by oral health inequities.

    Content:

    The Advancing Equity Through Oral Health Fund supports work that:

    • Seeks to improve access to equitable care through federal and/or state oral health policy
    • Contributes to the development of new, innovative medical-dental integrated care models and/or a strong and diverse integrated health care workforce

    The Emergent & Relief Fund supports:

    • The ability of communities and partner organizations to provide immediate relief due to unforeseen circumstances (e.g. community displacement due to a natural disaster)
    • The response to urgent and unexpected clinical issues that impact the direct access to care, particularly for historically marginalized communities (e.g. sudden equipment failure)

    Step 2: Submit a Concept

    All applicants to any of the CareQuest Institute funds are invited to initiate the grant submission process by submitting a concept of a proposed project, up to 500-words.

    Within the concept, CareQuest Institute requests that applicants include:

      1. Description of the issue you intend to address
      2. Description of the project for which you are seeking funding
      3. How the requested funds will be used to support the successful execution/implementation of the project

        Organizations can submit concepts at any time through our online grants management portal, Fluxx. After review, we will invite organizations whose work strongly aligns with CareQuest Institute’s priority areas to enter the next stage of the application process, to submit a full proposal.

        If you do not have a Fluxx account, please register and set up an account. You will receive a username and password to log in to the portal within three business days.

          Step 3: Submit a Full Grant Proposal

          Upon invitation, applicants can submit a full proposal, which is a more comprehensive application that includes a project narrative, work plan, and budget. This detailed proposal allows applicants to fully describe the proposed scope of work and corresponding steps necessary for implementation.

            Please note that in 2026, CareQuest Institute will transition from four grantmaking cycles per year to three, and in 2027 and beyond, it will move to two cycles per year. More information will be provided to our existing grantees about the change in late 2025. However, existing grantees with questions may also reach out to their Grants team contact for further assistance. Dates are subject to change.
